The Patient

A 58-year-old female with ongoing right foot pain.

She was diagnosed with right Posterior Tibialis Tendon Dysfunction.

She needs an orthotic that would fit in various footwear coupled by a rehabilitation and strengthening programme.

 

Prescribed Orthotic Type

Dress Standard Slimline (Direct Milled) Device – a slimline ladies’ device which would fit most ladies’ slip-on casual and dress shoes with heel heights less than 3 cm. Direct milled was chosen for this patient as it is more robust and cost-effective.

 

Lab Standard Description

  • Semi-rigid polyproline shell. Black polypropylene comes as standard in dress devices as it is more discreet. Direct milled products can only be polypropylene or Delrin. The thickness of the shell will depend on the patient’s weight and the rigidity specified.
  • 8-10 mm shell; this provides some control without taking up too much room in the heel counter
  • Moderate slimline (hourglass) grind; this will make finding suitable footwear to accommodate the orthotic easier.
  • Shallow HD EVA extrinsic rear-foot post with durasole thin strike plate; this is also to reduce bulk at the heel to prevent slipping where possible.
  • Metatarsal length is ideal for dress devices as it leaves space in the toe box

1.5mm Puff top cover: offers cushioning, is durable, this surface helps keep the foot stable within footwear, easy to clean.

2mm heel lift bilaterally; to accommodate osseousis equinus

Narrow shell grind; accommodate device in a variety of footwear
